Questionnaire ratings of attention-deficit/hyperactivity disorder (ADHD) in adults are associated with spatial working memory

Objective
Data related to brain function may have the potential to improve the reliability and validity of assessments for the aetiologically and clinically heterogeneous syndrome of attention-deficit/hyperactivity disorder (ADHD). This study investigated associations between questionnaire assessments of behavioural features of adults with ADHD and an aspect of neurocognitive performance which has been reported to be impaired in adults with ADHD.
MethodsFifty-nine adult patients with a DSM-IV diagnosis of ADHD, and their informants, completed questionnaires related to aspects of severity of ADHD. Associations were examined between questionnaire ratings and performance on a computer-administered task of spatial working memory (SWM).
ResultsCorrelations between ratings of ADHD and SWM indicated moderate but significant correlations for patients' ratings, but not for informants' ratings. Also, patients who reported a past history of ‘self-harm’ (N = 33) had a significantly worse mean performance on both measures of SWM (p = 0.004, 0.003).
ConclusionsThe results indicate that aspects of impulsivity, i.e. self-ratings of ‘emotive’ behaviour (involving rapid response to stimuli and marked reactivity of mood) and of past ‘self-harm’, show relatively strong associations with SWM performance in adults selected on the basis of an ADHD diagnosis. A profile of neurocognitive performances may have a role in the assessment of ADHD.

Polypharmacy is growing in Canada, along with adverse drug events and drug-related costs. Part of the solution may be deprescribing, the planned and supervised process of dose reduction or stopping of medications that may be causing harm or are no longer providing benefit. Deprescribing can be a complex process, involving the intersection of patients, health care providers, and organizational and policy factors serving as enablers or barriers. This article describes the justification, theoretical foundation, and process for developing a Canadian Deprescribing Network (CaDeN), a network of individuals, organizations, and decision-makers committed to promoting the appropriate use of medications and non-pharmacological approaches to care, especially among older people in Canada. CaDeN will deploy multiple levels of action across multiple stakeholder groups simultaneously in an ecological approach to health system change. CaDeN proposes a unique model that might be applied both in national settings and for different transformational challenges in health care.

In(IL; IR)O Y348 is a pericentric inversion of linkage group I in N. crassa, with a breakpoint between fr and un-5 in the left arm and a breakpoint between ad-9 and nit-1 in the right arm. Approximate breakpoint location was found by tabulating crossovers between the rearrangement and markers in normal chromosome sequence. Inversion structure was verified by marked In O Y348 × In O Y348 crosses. Precise mapping of breakpoints was by duplication coverage. Inversions like O Y348 do not produce progeny with segmental chromosome duplications when crossed to normal sequence, but duplications were produced by crossing it to In(IL; IR)O Y323 (Barry & Leslie, 1982), another standard pericentric inversion, and to T(I → VI)NM103 (Turner, 1977), a translocation to a tip. Each of these rearrangements has a breakpoint within the inverted region of In O Y348. Two duplications from In O Y348 × In O Y323 were converted to normal chromosome sequence by double mitotic recombination. Besides expediting mapping, the technique of intercrossing rearrangements increasingly enables us to make segmental duplications exactly tailored for studying specific included genes.
Relationship between psychiatric disease and neuropsychological impairment in HIV seropositive individuals

Neuropsychological impairment and DSM-III-R Axis I psychiatric diagnoses were evaluated in a heterogenous group of HIV seropositive individuals and seronegative individuals with similar risk factors for HIV infection. Neuropsychological and psychiatric disorders were common in the HIV seropositive group, but there were no relationships between these two aspects of neuropsychiatric dysfunction in seropositive patients. Results indicate that psychiatric disorders in HIV seropositive individuals tend to predate infection and decrease over time following knowledge of seroconversion, suggesting that they are primarily a function of psychosocial factors. Neuropsychological disorders are specific to HIV seropositive patients and tend to increase over time following seroconversion, suggesting that they are due to neurological effects of HIV-infection. Introduction and Overview
Numerous neuropsychiatric disorders, such as attention deficit hyperactivity disorder (ADHD), schizophrenia, frontotemporal dementia and Parkinson's disease, are characterized by cognitive impairments. The potential public health benefit of improving current treatments for cognitive disabilities is undisputed (Meltzer, 2003). The disorder of schizophrenia illustrates particularly well the need for specific treatments of cognitive dysfunction. The clinical targets for schizophrenia have traditionally been the delusions and hallucinations that characterize the disorder, and for which neuroleptic agents are most efficacious (Campbell et al., 1999). However, although these symptoms define the core diagnostic criteria of this illness, it is now recognized that the cognitive and motivational impairments in schizophrenia are closely related to the profound long-term disability typically produced by the disease (Hyman & Fenton, 2003). Indeed, cognitive deficits are the major cause of poor psychosocial function and impoverished quality of life in patients with schizophrenia for whom psychosis is controlled by currently available therapies (Geyer & Tamminga, 2004).
Neuropsychological studies of patients with schizophrenia have consistently identified deficits on tests of executive function, known to be sensitive to frontal lobe damage. Deficits in cognitive flexibility, working memory and planning have been widely reported, together with more recent evidence of impairments in inhibitory control (Badcock et al., 2002; Pantelis et al., 1999; Elliott et al., 1995; Pantelis & Brewer, 1995). Moreover, cognitive flexibility, as measured by the Wisconsin Card Sorting Test (WCST), has been related to community outcome in these patients (Hartman et al., 2003; Koren et al., 1998; Green, 1996) and to response to psychological intervention (Wykes et al., 2003).

The prospect of being able to take safe and effective drugs to improve mental functioning is becoming a reality. With the potential for widespread use of cognitive enhancers by large sectors of the population, this article discusses the rationale behind the development of these drugs and how society might benefit from them. Important ethical questions and scenarios are also raised. Scientists are urged to explore the implications of their work and engage in active debate with a wide range of interested stakeholders about the ethical and moral consequences of these new technologies to ensure maximal benefit with minimal harm.
